Blepharoplasty in Delhi: eyelid surgery, explained

Hooded upper lids and under-eye bags are structural, not tiredness. What eyelid surgery does, and what it does not.

  • Upper lid surgery removes excess skin; lower lid surgery usually repositions fat rather than removing it.
  • Blepharoplasty corrects the eyelid itself. It does not lift the brow or erase crow's feet.

  • Done badly, lower lid surgery pulls the lid down. Conservative technique matters more here than almost anywhere on the face.

Why the eyes age first

The skin of the eyelid is the thinnest on the body, and the eye is the first place anyone looks. Both facts conspire to make the eyelid the earliest region to show age, often a decade before the jaw does.

Two separate things happen. The upper lid loses skin elasticity and begins to hood, sometimes far enough to rest on the lashes. The lower lid loses the support that holds orbital fat behind a thin restraining ligament, and that fat begins to bulge forward, which is what people call a bag.

These are structural changes. No cream reaches the layer where they happen, which is why the results of eye creams are so consistently disappointing on this specific complaint.

What upper eyelid surgery does

Upper blepharoplasty removes a measured strip of excess eyelid skin, and occasionally a small amount of fat or muscle. The incision sits in the natural crease of the lid, where it becomes very difficult to see once healed.

The effect is that the lid opens. People frequently report that applying eye makeup becomes possible again, or that the heaviness they felt reading at night has gone.

The commonest mistake in upper lid surgery is removing too much. A lid that cannot close fully is a far worse problem than one that hoods slightly.

What lower eyelid surgery does

Lower lid surgery is the more technically demanding of the two. Modern practice has moved away from simply cutting fat out, because removing the fat that gives the under-eye its fullness tends to produce a hollow, skeletal look that ages badly.

Instead, the fat is usually repositioned: released from where it bulges and redraped over the orbital rim, filling the groove beneath it. The bag and the hollow beneath it are treated as one problem rather than two.

Where there is little excess skin, the incision can be placed inside the lid, leaving nothing visible at all.

What it will not fix

  • A heavy brow. If the brow has descended, taking skin from the lid treats the symptom and can make the brow sit lower still. This needs a brow lift.
  • Crow's feet. These are dynamic lines from muscle movement, better addressed with botulinum toxin.
  • Dark circles from pigment. Shadowing caused by a hollow improves; pigment in the skin itself does not.
  • Fine crepey skin. Often better served by resurfacing than by surgery.

An honest consultation will tell you which of these you actually have. Frequently people arrive asking for eyelid surgery and leave understanding that a brow lift is the operation that will change what is bothering them.

Recovery, realistically

Bruising and swelling are expected and peak around the second or third day. Stitches typically come out at five to seven days. Most people feel comfortable in company at around two weeks, though fine swelling can take a few months to fully resolve.

Vision is often blurry for a day or two from ointment and swelling. Reading and screens are tiring in the first week. Plan for that rather than fighting it.

Common questions

Is blepharoplasty painful?+

Most people describe discomfort rather than pain, controlled with simple analgesia. The eyelid has few deep pain fibres. Tightness and dryness are more common complaints than pain.

Will eyelid surgery make me look different?+

It should make you look rested, not altered. The shape of the eye is preserved; only the excess is addressed. If someone cannot place what has changed about you, the surgery has worked.

How long do the results last?+

Upper lid results commonly last ten years or more. Lower lid results are usually longer still, because repositioned fat does not tend to return. Ageing continues, but you do not return to the starting point.

Can it be combined with a facelift?+

Yes, and it often is. A facelift addresses the cheek, jaw and neck but does not change the eyelid. Combining them treats the face as a whole rather than in isolated regions.
